Van Slooten, Jacob C. “Jake” Greece/Florida: Saturday, September 16, 2017 at age 61. Survived by his mother, Mary Van Slooten; wife, Maggie Van Slooten; her, children, Rachael and Ruben Almeida; his siblings, Debbie (Patrick) LaMendola and Charlie Jr. (Laura) Van Slooten; nieces and nephews, Nick LaMendola, Kellie (Ryan) Dunn, Erica (Furkan) Ozen and Brandon Van Slooten, Trisha (Efrain), Audrey (Robert), Tim (Samantha), Tom (Natalie) brother and sister in-law, Tim (Karen) Murphy; many dear friends including Beverly Lyness. Predeceased by his wife, Karen; father, Charles H. Van Slooten. Friends may call Wednesday (Oct 4th) 4-6 PM at Vay-Schleich and Meeson Funeral Home, 1075 Long Pond Road.
